One of the greatest gifts you can give your loved ones is clear direction on your end-of-life preferences.  No one wants to talk about death, but it’s a reality for us all.  You don’t want to keep them guessing.  More importantly, you don’t want them to feel guilty about making the right decision.  That’s why it’s great to have a frank discussion about what’s coming and how to document your wants and needs.

  • How to have the difficult discussion with the patient and family members; and getting everyone on the same page.
  • Choosing a health care advocate.
  • Determining what types of care are acceptable.
